Terms and Conditions of Business and Client:
Personal Trainer agreement

Definitions

1. ‘I’ or ‘me’ or ‘my’ means Neil Forsyth.
2. ‘You’ or ‘your’ means you.

General

1. I will use my skills and knowledge to design safe programmes of exercise and provide the coaching, supervision, advice and support that you may need to help achieve your personal goals.
2. Your progress will be regularly monitored and the programme revised and adjusted accordingly.
3. I will provide all necessary equipment and will organise appropriate venues for all training sessions.
4. All your information will be kept strictly private and confidential and held securely in accordance with the Data Protection Act.

Medical Matters

1. You will be required to complete a Physical Activity Readiness Questionnaire [‘PAR-Q’] form before starting a training programme.
2. You may be required to obtain a letter of medical clearance from your GP before starting a training programme. Please be aware that your GP may charge you for this.
3.  If I require further medical information from a practitioner (which includes your GP), you must  provide  such details and authorise the practitioner to release such medical information to me. Please be aware that your GP may charge you for this.
4. If at any stage I have any concerns about your medical health, I reserve the right to cease training and refer you back to your practitioner.

Training Sessions

1. It is understood that both you and I will commit to the programme and give 100% effort. You understand and agree that you will follow the advice given by me (including any nutritional and lifestyle advice), in order to achieve your goals.
2. You are required to arrive 5 minutes prior to a training session so that a full session can be achieved on each visit.
3. You are required to wear appropriate clothing and footwear.  Clothing should be supportive and provide adequate ventilation. Footwear should be comfortable and provide adequate support.
4. It is your responsibility to attend training sessions having consumed adequate and appropriate nutrition and water. No alcohol is to be consumed beforehand on the day of any training session.  

Lateness Policy 

1. If I am less than 10 minutes late, I shall be required to still take the session for the full length of time.
2. If I am more than 10 minutes late or I have to cancel the session with less than 24 hours’ notice, you will be entitled to a complimentary session as well as the missed or cancelled session.
3. If you are less than 15 minutes late, you shall still have the remaining time of the session, i.e. if you are 15 minutes late you shall receive a 45 minute session not an hour.
4. If you are more than 15 minutes late I am entitled to leave the premises and you will lose the session and will be liable for full payment of the session fee.

Cancellation Policy 

1. If you cancel a session with less than 24 hours’ notice you shall lose the session and will be liable for full payment of the session fee.
2. If I cancel a session within 24 hours of it taking place, you shall receive a complimentary session as well as the cancelled session.

Multi Session Personal Training and Block Class Booking Expiry

1. Single sessions must be used within 4 weeks of purchase date.
2. Online training packages will last the duration of time stated.
3. 6 session/class packages must be used within 12 weeks from first session/class.
4. 10 session/class packages must be used within 20 weeks from first session/class.
5. Special packages must be used on the specific date or dates of the package.

Fees

1. See fees section of my website for fee structures and full payment terms.
2. Single sessions must be paid in full at the end of each session.
3. Single online and/or telephone advice/training must be paid in full within 14 days.
4. Single class fees must be paid in full at the end of each class.
5. All multi session and special packages (including multi online and telephone advice/training, block class bookings, 6 or 10 session personal training bookings), must be paid in full in advance.
6. Payment by cash, cheque or electronic transfer is accepted.
7. All monies paid are non-refundable.
